Health
care delivery is an essential service that contributes significantly to the
overall well-being and productivity of individuals and the society at large.
Hospitals, being the main institutions of health care provision, play a vital
role in ensuring that patients receive timely and adequate medical attention
(Adebayo, 2020). Generally, hospital services are categorized into outpatient and inpatient care. Outpatient services involve patients who visit the
hospital for diagnosis or treatment but do not stay overnight, while inpatient
services involve patients who are admitted and stay for at least one night for
continuous medical attention (Okeke, 2019).
Understanding
the pattern and nature of outpatient
and inpatient attendance is important in evaluating hospital
performance, resource allocation, and quality of care. The increasing demand
for outpatient services has become a global trend due to advancements in
medical technology, shorter treatment times, and efforts to reduce hospital
costs (World Health Organization [WHO], 2021). Conversely, inpatient services
remain indispensable for cases that require close medical supervision, surgical
interventions, or long-term treatment.
In
Nigeria, the health system is structured across three tiers: primary, secondary, and tertiary levels,
with general hospitals such as Dutse
General Hospital serving as secondary health facilities. These hospitals
provide both outpatient and inpatient services to communities, acting as a
bridge between primary health care centers and teaching hospitals (Federal
Ministry of Health [FMOH], 2020). Despite government investments, challenges
such as limited manpower, inadequate facilities, and uneven patient
distribution between inpatient and outpatient departments persist (Afolabi,
2018).
In Dutse General Hospital, the number of
outpatients has continued to rise over the years due to increased awareness,
accessibility, and affordability of medical services. Meanwhile, inpatient
admissions are often influenced by the severity of diseases, available bed
space, and the hospital’s capacity to handle serious cases. Comparing
outpatient and inpatient attendance patterns helps to identify service gaps,
resource utilization efficiency, and the effectiveness of hospital management
strategies (Lawal & Musa, 2019).
This
study, therefore, aims to carry out a comparative
analysis of outpatient and inpatient attendance at Dutse General Hospital, highlighting
trends, influencing factors, and implications for health service delivery in
Jigawa State.
1.2 Statement of the Problem
In many Nigerian hospitals, there is
an increasing disparity between outpatient and inpatient attendance. While
outpatient departments are often overcrowded, inpatient facilities may be
underutilized or overstretched depending on the nature of medical cases (Bassey,
2021). Dutse General Hospital, being one of the major secondary health
facilities in Jigawa State, has experienced fluctuating patterns of patient
attendance, which can affect hospital planning, staff workload, and quality of
service delivery.
The hospital
records show that outpatient visits have increased significantly over the past
few years, while inpatient admissions have either stagnated or declined. This
imbalance may result from factors such as cost of admission, patients’
preference for quick treatment, limited bed capacity, or improved outpatient
care. However, there is limited empirical data comparing these patterns to
determine their causes and implications.
Thus, the
major problem of this study is the lack
of systematic analysis of outpatient and inpatient attendance data in
Dutse General Hospital, which could provide insight into how health services
are utilized and guide better decision-making for hospital management and
policy formulation.
1.3 Objectives of the Study
The main objective of this study is
to conduct a comparative analysis
of outpatient and inpatient attendance at Dutse General Hospital.
The
specific objectives are to:
- To examine the pattern of outpatient
attendance at Dutse General Hospital.
- To assess the pattern of
inpatient attendance at Dutse General Hospital.
- To exmine weather there is
significant difference of patient attendance base on gender.
This study seeks to answer the
following research questions:
- What are the trends and
patterns of outpatient attendance at Dutse General Hospital?
- What are the trends and
patterns of inpatient attendance at Dutse General Hospital?
- What factors influence
outpatient and inpatient attendance?
- What challenges affect
outpatient and inpatient services at the hospital?
- How can hospital management
improve service delivery in both categories?
The following hypotheses were
formulated for the study:
- H₀₁: There is no significant
difference between outpatient and inpatient attendance at Dutse General
Hospital.
- H₀₂: There is no significant
relationship between demographic characteristics of patients and their
choice of outpatient or inpatient services.
This research is significant because
it provides a better understanding of hospital service utilization in secondary
health facilities in Nigeria. The findings will help hospital administrators in
planning and resource allocation,
ensuring balanced attention to both outpatient and inpatient services (Obi,
2017).
Furthermore,
policymakers in the Jigawa State
Ministry of Health can use the results to design effective strategies
that enhance hospital efficiency and patient satisfaction. The study will also
serve as a reference material for
researchers and students interested in hospital management, health
service evaluation, and public health research.
1.7 Scope and Limitations of the
Study
This study is limited to Dutse General Hospital, Jigawa State,
focusing on outpatient and inpatient attendance records from 2020 to 2024. It covers trends,
influencing factors, and challenges related to hospital utilization.
Limitations encountered include
difficulty in obtaining accurate hospital records, time constraints, and the
limited number of respondents due to hospital workload. Despite these
challenges, adequate data were gathered to ensure validity and reliability of
findings.
1.8 Definition of Key Terms
Ø Outpatient: A patient who visits the hospital
for medical consultation or treatment without being admitted overnight (WHO,
2021).
Ø Inpatient: A patient who is admitted into a
hospital and stays for at least one night for medical treatment or observation
(FMOH, 2020).
Ø Attendance: The record of patients who receive
medical services either as inpatients or outpatients.
Ø Hospital
Utilization:
The extent to which hospital facilities and services are used by patients over
a given period.
Ø Comparative
Analysis: A method
of evaluating two or more items by comparing their similarities and differences
in measurable aspects.
